At First Colony Aquatic and Rehabilitation Center, we want to maximize your time with us. Prior to your first visit, please print and complete the New Patient Forms below. Please bring these completed forms, your insurance cards, and photo ID with you to your first visit. Otherwise, allow extra time before your appointment to fill them out here.

Direct Access

Texans No Longer Need a Referral to be Treated by a Physical Therapist

Beginning September 1st Texans who want physical therapy for an injury or pain management can now access their Physical Therapist without having to first go see a physician, or other healthcare provider.

The Texas Legislature earlier this year passed HB 29 by Rep. Ina Minjarez (D-San Antonio). HB 29 allows Texans to seek the treatment they need without the delay and expense of having to see a physician first. People can now go directly to their Physical Therapist and be treated for up to 10 business days (15 in some instances).

At First Colony Aquatic and Rehabilitation, we accept most major insurances, including Medicare, Tricare, VA, and Workers’ Comp, Aetna, Blue Cross/Blue Shield, United Healthcare, Ambetter, and Community Health Choice.

As a courtesy to our patients, we will verify your health benefits prior to your arrival to determine if there will be any copayment, deductible, or coinsurance for therapy services.

What is physical therapy?

Physical therapy is a medical, hands-on approach to relieving pain and restoring function. Physical therapists are medical experts in musculoskeletal and neurological conditions and work closely with physicians. Our physical therapists evaluate your movement patterns, identify the cause of your pain, and work to eliminate it.

While every patient’s condition and pain is unique, your physical therapist will use a variety of gentle hands-on techniques, exercises, and equipment to relieve pain quickly and restore your function to optimum levels. The overall goal is to restore pain-free mobility and function and to prevent the injury or pain from recurring.
